Please note the following:

This form, the pet's medical records, and the client completed Behavior Questionnaire must be submitted by the Saturday before each webinar.
Only one case will be selected each week. There is no guarantee your case will be selected.
If your case is selected, you will receive a confirmation email with additional instructions and information.
As a selected contributor, you will be invited to the webinar as a "co-host," so you can interact directly with the provider via video and microphone.
If your case is not selected for the webinar, you still have the option to schedule a private appointment with the provider to discuss your case.
Your client is not invited to the webinar. This is for pet professionals only. You are responsible for providing the information directly to the pet owner.
